Our One-to-one Outreach Service

What does this service focus on?

This service focuses on overcoming barriers the individual is facing in relation to their independence, their ability to take part in community life, their capacity to return to a form of employment or education, and their capability to manage their symptoms of brain injury in day-to-day life.

How does it work?

Our Occupational Therapists will carry out an assessment of your needs and create a person-centred care plan identifying your goals for the Key Support Worker to support you with:

• Home based: re-learning of activities of daily living.

• Home based: techniques and tools for organising/sequencing the day, budgeting/paperwork/making appointments/using IT for work or social requirements.

• Community based: use of public transport, exercise, accessing local assets and building social connections to reduce isolation.

• Return to work/volunteering or study: support with CV creation, searching for appropriate roles, understanding what reasonable adjustments can be made by employers, techniques for coping with fatigue/emotions and mood etc in the workplace.

Please note the things we do not do in outreach sessions:

• Personal care

• Administering medications

• Home / domestic care

Please note, this service is a chargeable services, however we can help refer your family member or friend for a Social Services assessment, as they may be eligible for funding from the council to access our support services.

Rates

Outreach sessions are usually booked in 2-hour slots, but additional hours can be added:

2 Hour Session= £48.33

Additional Rate per Hour= £23.47

This is subject to additional travel or other expenses which will be identified and applied on a case by case basis depending on the location of the client.

This service can either be paid for privately OR funding can be applied for under the Care Act via Adult Social Services and the local County Council.
